INSTRUCTIONS: Please Read Carefully
1. To establish care with us, the quickest way to complete our intake paperwork is to stop by our office Monday-Friday between 10am-12pm or 1pm-4pm. The process should take less than 20 minutes and you will be able to schedule an appointment before you leave. Please bring your insurance card and photo ID.
2. Completing paperwork online means you will also need to take a picture of your photo ID and insurance card, upload and attach it while completing the online process via PDF Filler, or fax a copy of your photo ID and insurance card to (360) 539-1715 in Lacey or to (360) 846-1722 in Longview. Please do not email insurance cards or information as email is not a secure method of transmission. If you do not have access to a fax, please bring your cards by our office for scanning.
3. Once we have all the required forms and have verified your insurance coverage, we will call you to schedule an initial assessment.
4. Completing these forms on a mobile device will be difficult, and the ensuing problems may delay processing. In that case, please consider visiting our office as the process is much quicker in person.
5. Apple Health clients: please be sure to indicate which managed care plan you are enrolled in and provide the subscriber information for that specific plan. Provider One information is not sufficient.
Notice:
1. Due to Washington health care laws, a Release of Information should be completed by clients age 13 and up if a parent or other party will need to communicate with us about scheduling or other information.
2. We are not able to provide any court-ordered assessments or services for chemical dependency, anger management, family court, or those with open CPS cases.
3. We do not prescribe or manage medication currently at any of our clinics.
